Module I: Endocrinology, a core course in the Fellowship, is critical in helping to develop the necessary skills to treat complex patients and cases. This module will focus on the hormonal changes that manifest in both men and women through the aging process, in addition to a review of patient case studies and interactive discussion with leading hormone experts. Sessions will review the understanding of the physiology and biochemistry of sex hormones, coupled with learning protocols for hormone dosing.

Learning Objectives

  • Interpret the structure of the sex hormones & their metabolism
  • Review the hormonal changes that manifest during the aging process, including functions of estrogen, progesterone, testosterone & DHEA in the body
  • Recall the importance of measuring hormonal tissue levels, in diagnosing hormonal imbalances & in monitoring bio-identical hormone replacement
  • Discuss the methodology of scientific support for & the differences in, saliva, urine & blood testing of hormones
  • Review the literature on synthetic & bio-identical hormone replacement
  • Assess treatment, dosages, and side effects of BHRT
  • Review key signs & symptoms of adrenal fatigue and relevant lab testing
  • Evaluate the differences between specialized compounding and manufacturing medications
  • Discuss the various patient care areas that involve specialized compounding
  • Discuss roles of iodine in the body, and treatment of iodine deficiency
  • Explain the effects estrogen has on the lipid profile for women
  • Review the effects estrogen has on post-transcriptional and translation modulation of proteins and enzymes for women
  • Examine the actions estrogen has on mitochondrial function in the vascular endothelium in women
  • Review the medical literature on the anti-inflammatory effects of estrogen for women
  • Review the medical literature on the effects that melatonin has on the heart in both males and females
  • Recall the effects that DHEA has on the heart in women and men including elevated DHEA levels negatively impacting intima-media thickness in men with type II diabetes
  • Recognize psychiatric symptoms that are related to sex hormone balance in both men and women
  • Discuss the psychiatric symptoms related to hypothyroidism
  • List the effects of enzyme inhibitors, receptor antagonists and hormonal agents which are resistant to metabolism
  • Review the physiology and biochemistry of thyroid function 
  • Discuss treatment strategies for patient with hormonally-related signs and symptoms
  • Recognize the interplay between sex hormones and thyroid hormones
  • Review the steroidogenic pathway
  • Recall the relationship between insulin, sex hormones, adrenal, and thyroid hormones
  • Describe new treatment modalities for patients with multiple diseases processes including hypothyroidism, insulin resistance, menopause, andropause, PMS, PCOS, infertility, hyperadrenalism, and hypoadrenalism
  • Review conventional & metabolic treatments for PCOS
  • Review the basics of genetics and SNPs
  • Review which SNPs affect estrogen metabolism
  • Discuss which SNPs affect the risk of breast cancer
  • Review testing that needs to be done for the breast cancer patient
