Course Description

In integrative medicine there is no single solution to a difficult clinical problem. Clinical Practice Protocols is designed to teach learners how to think for themselves about complex cases. This module is designed to encourage reasoning and decision making. Personalized medicine is unique to each patient, there is no cookbook of recipes to follow.

Module 8 is a culmination of the first 7 modules. The learning atmosphere will encourage decision making to happen amongst colleagues and learners. There will be no right answer. The groups themselves will have various levels of expertise and knowledge allowing for learners to explore their own methods of treating patients effectively

Learning Objectives

Module I Endocrinology

  • Recognize the hormonal changes that women & men manifest with aging and discuss the functions of estrogen, progesterone, testosterone & DHEA in the body
  • Synthesize knowledge regarding hormone replacement therapies in men and women, to conduct case analysis of complex hormonal imbalances including PMS, PCOS, and menopause and andropause
  • Demonstrate mastery of managing patients related to age-related and premature hormone decline, hormone metabolism and disposition, epigenetics, and environmental influences on endocrinopathies
  • Apply learning to patients with obesity, insulin resistance and diabetes to correct dysglycemia due to poor dietary habits, nutrient deficiencies, hormonal imbalances and toxic exposures.
  • Discuss the differences between compounding & manufacturing medications and review the various licenses needed to compound medications

Module II Cardiology

  • Discuss accepted and emerging risk factors for heart disease including elevated cholesterol, triglycerides, lipid fractionation, homocysteine, lipoprotein (a), ferritin, fibrinogen & c-reactive protein
  • Apply advanced lipid testing (lipid particle number and size and HDL functionality and quality) for the diagnosis and treatment of dyslipidemia in clinical practice.
  • Analyze cases of CVD, CHD and renal disease Mutilizing the model of inflammation, oxidative stress and vascular immune dysfunction to prevent and treat atherosclerosis utilizing lifestyle, nutritional, nutraceutical and metabolic medical therapies

Module III Neurology

  • Discuss major neurotransmitter activity in the nervous system & their functional roles in behavior & neurological conditions
  • Synthesize knowledge regarding exposures, biological toxins, and their impact on the neuroimmune and gut-neuro axis including innate immune pathology, dysautonomias and structural brain related changes in chronic mood and cognitive disorders.
  • Apply new functional and integrative treatment modalities for sleep disorders, multiple sclerosis, stroke recovery, Parkinson's Disease, ALS & Alzheimer's disease and related conditions

Module IV Gastroenterology

  • Apply concepts of leaky gut, dysbiosis, hypochlorhydria, and immune intolerance in GI related conditions
  • Properly diagnose and treat GERD, peptic ulcer disease, irritable syndrome and food sensitivities utilizing a metabolic and nutritional approach in selected cases.
  • Demonstrate ability to utilize new diagnostic tests including DAO, LPS, zonulin, microbial, and immunologic panels in common GI disorders
  • Solve complex gastroenterology cases that represent key systems biology relationships to the nervous and immune systems

Module V Nutrition and Exercise Prescription

  • Understand the key factors in fat loss and a practical way to identify disruptors of metabolic efficiency and reserve. This discussion to include labs, biometrics and subjective data.
  • Apply a personalized exercise prescription plan with practical lifestyle therapies, graded fitness recommendations and discuss integrated patient profiles drawn from all modules of the fellowship.
  • Demonstrate proficiency for utilizing proper biomechanical evaluation in the trained and untrained athlete
  • Introduce and explain the concept of nutrition periodization, metabolic efficiency training, compounded prescriptions and dietary supplementation

Module VI Toxicology

  • Know the mechanisms of toxic injury including disturbances in cell signaling, alterations in structural entities such as the mitochondria & impaired synthesis of specific molecules such as fatty acids, proteins, nucleotides, glutathione & phospholipids
  • Demonstrate ability to Identify and remove toxic heavy metals and organic pollutants in selected patient cases, utilizing lifestyle factors, dietary supplements, supportive modalities and selected medications while protecting key organs of detoxification
  • Apply knowledge of biological toxins to complex patient presentations to address and correct neuroimmune dysfunction, spectrum disorders, neurocognitive decline and neurologic diseases

Module VII Immunology

  • Apply a functional and metabolic model to auto-immune conditions by assessing environmental triggers, nutrient deficiencies, hormonal imbalances, microbial load and dysbiosis, stress and poor lifestyle choices.
  • Demonstrate understanding of the cellular & molecular biology of immunity, factors that lead to loss of immune tolerance, and methods to restore normal immune function in Parkinson’s Disease, Mulitple Sclerosis and Alzheimer’s Disease.
  • Synthesize and apply concepts related to assessment and management of acute and chronic inflammation in clinical cases and utilize Lifestyle, metabolic and functional modalities in treatment planning

Jill Carnahan, MD, ABFM, ABIHM, completed her residency at the University of Illinois Program in Family Medicine at Methodist Medical Center. In 2006 she was voted by faculty to receive the Resident Teacher of the Year award and elected to Central Illinois 40 Leaders Under 40. She received her medical degree from Loyola University Stritch School of Medicine in Chicago and her Bachelor of Science degree in Bio-Engineering at the University of Illinois in Champaign-Urbana. She is dually board-certified in Family Medicine (ABFM) and Integrative Holistic Medicine (ABIHM).

She was also part of the first 100+ health-care practitioners to be certified in Functional Medicine through the Institute of Functional Medicine (IFMCP). In 2008, Dr. Carnahan’s vision for health and healing resulted in the creation of Methodist Center for Integrative Medicine in Peoria, IL where she served as the Medical Director for 2 years. In 2010, she founded Flatiron Functional Medicine in Boulder, Colorado where she partnered in functional medicine with medical partner, Dr. Robert Rountree. She recently opened a brand new medical clinic with a broad range of services in Louisville, Colorado. Dr. Carnahan is also 14-year survivor of breast cancer and Crohn’s disease and passionate about teaching patients how to “live well” and thrive in the midst of complex and chronic illness. She is also committed to teaching other physicians how to address underlying cause of illness rather than just treating symptoms through the principles of functional medicine. She is a prolific writer, speaker, and loves to infuse others with her passion for health and healing!

MD, MHSA

Andrew Heyman, MD, MHSA is an internationally recognized expert in Integrative Medicine. He is currently the Program Director of Integrative and Metabolic Medicine at The George Washington University. Prior to assuming this role, he spent 16 years at the University of Michigan, serving to build one of the largest and most successful academic-based Integrative Medicine programs in the United States.

In 1990, he received formal training in Five-element Shiatsu and Traditional Chinese Medicine, during which time he developed an interest in increasing access of natural therapies to underserved populations while partnering with the University of Pennsylvania. He continued this work with Jim Gordon, MD at the Center for Mind-Body Medicine in Washington DC exploring the intersection between public health, at risk groups, and alternative therapies.

Dr. Heyman eventually went on to receive a Masters in Health Services Administration at the University of Michigan School of Public Health and upon graduation, became the administrator for the University of Michigan Complementary and Alternative Medicine Research Center where he was responsible for administering a $7 million NIH grant to research alternative therapies for cardiovascular diseases. The center was one of the first in the country to place alternative therapies and their scientific examination in an academic context.

Dr. Heyman has held several leadership positions in the field of Integrative Medicine. He was the national clinical working group co-chair of the Consortium of Academic Health Centers for Integrative Medicine for 4 years, a collaboration of 57 North American universities involved in Integrative Medicine. He is the online editor for the Journal of Men's Health in the section of Integrative Medicine, and he also holds the position of Editor-In-Chief of the Internet Journal of Anti-Aging and Aesthetic Medicine. His research interests include the Stress response and NeuroEndocrinology, Cardiometabolic Disease, Men's Health and clinical outcomes research methodologies.

Dr. Heyman remains clinically active as well, as the owner of a four-physician integrative medicine practice in Northern Virginia at the Virginia Center for Health and Wellness. He is also the President of Integrative Health Resources, a small group of clinical experts that consults to physicians, health systems and public health professionals seeking to develop wellness, lifestyle and nutrition programs. Finally, Dr. Heyman has been featured on the Discovery Channel, Martha Stewart Living Magazine, Ladies Home Journal, Natural Health Magazine and appeared in many other TV and print venues and is a widely sought after, and celebrated speaker around the world on many Integrative Medicine topics. His teaching approach is described as deeply rooted in scientific evidence, grounded in the clinical reality of expert patient care, and readily accessible to both the new learner and seasoned practitioner alike.

Mark C. Houston, MD, MS, ABAARM, FACP, is Associate Clinical Professor of Medicine at Vanderbilt University School of Medicine, Clinical Instructor in the Department of Physical Therapy and Health Care Sciences at George Washington University (GWU), Director of the Hypertension Institute and Vascular Biology and Medical Director of the Division of Human Nutrition at Saint Thomas Hospital in Nashville, TN.

Dr Houston was selected as one of the Top Physicians in Hypertension in the US in 2008-2014 by the Consumer Research Council, and by USA Today as one of the Most Influential Doctors in the US in both Hypertension and Hyperlipidemia twice in 2009- 2010. He was selected as The Patient’s Choice Award in 2010 -2012 by Consumer Reports USA. He is triple boarded certified by the American Board of Internal Medicine (ABIM), the American Society of Hypertension (ASH) (FASH) and the American Board of Anti-Aging and Regenerative Medicine (ABAARM, FMNM). He holds two Masters of Science degrees in HUMAN NUTRITION from the University of Bridgeport, CT and another in METABOLIC and NUTRITIONAL MEDICINE (University of South Florida School of Medicine-Tampa). James LaValle, RPh, CCN, is a nationally recognized clinical pharmacist, author, board certified clinical nutritionist, founder of Metabolic Code Enterprises, Inc., a web platform and practice solution enterprise, launching AIR Support and the Metabolic Code Assessment. In addition, he founded Practices at Progressive Medical Center in Orange County, CA. In 2001, he founded and operated LaValle Metabolic Institute, an interdisciplinary medicine facility in Cincinnati for the past 15 years (sold in 2014) where he served thousands of patients using his metabolic model for health.

He also founded Integrative Health Resources 15 years ago, which is focused as a natural products industry consulting company. James has 27 years of experience integrating natural therapies into various medical and business model. James is probably best known for his expertise in natural therapeutics application and drug/nutrient depletion issues and uncovering the underlying metabolic issues that keep people from feeling healthy and vital. As such he has written hundreds of articles for a variety of industry journals and publications, and has lectured for thousands of healthcare professionals and consumer audiences globally on these topics. Pamela Wartian Smith, M.D., MPH, MS spent her first twenty years of practice as an emergency room physician with the Detroit Medical Center and then the next twenty years as an Anti-Aging/Functional Medicine specialist. She is a diplomat of the Board of the American Academy of Anti-Aging Physicians and is an internationally known speaker and author on the subject of Metabolic, Anti-Aging and Functional Medicine. She also holds a Master’s in Public Health Degree along with a Master’s Degree in Metabolic and Nutritional Medicine.

She has been featured on CNN, PBS, and many other television networks, has been interviewed in numerous consumer magazines, and has hosted two of her own radio shows. She is a regular contributor for Fox News Radio. Dr. Smith is currently the Director of the Center for Personalized Medicine and the founder of The Fellowship in Anti-Aging, Regenerative, and Functional Medicine. Dr. Smith is also the co-director of the Master’s Program in Metabolic and Nutritional Medicine at the Morsani College of Medicine, University of South Florida.
